You will need to register each child separately (if more than one) before they can attend AWANA's at Temple Baptist. It is a two step process. Please fill out the form below with general and required information about each child and click submit. You will then be redirected to the Temple Baptist Church Blanket Medical Release form page. Please be sure to fill it out in its entirety and click submit. Once again, both forms need to be filled out for each child that will attend AWANA's. No one will be allowed to participate in AWANA's until both forms are completed. Thank You!
